Frailty posters

The below posters were developed in collaboration with the Patient Information Forum (PIF). The Frailty and Surgery poster was developed with the Centre for Perioperative Care (CPOC).

They explain and illustrate the stages of frailty and what this means for them.

Think Frailty

This 'Think Frailty' poster was developed by Oxford University Hospitals NHS Foundation Trust as a teaching tool, to help staff to identify frailty and therapy amenable problems, and make them aware of the services and pathways available to support patients to live well with frailty after a hospital attendance.
