<p class="larger-text">The British Geriatrics Society are proud of the role we played in supporting our late friend, colleague and BGS member, Dr Kate Granger, with her <a href="https://www.hellomynameis.org.uk/">#hellomynameis campaign</a>. This approach has now become established as routine practice across the NHS. At the heart of Kate’s campaign lay an assertion, based upon her lived experience, that patients need to know who is treating them.</p>\r\n
\r\n
<p>Effective care of older people is multidisciplinary by nature and highly specialised. Delivering it to the highest standard demands that we attract, train and retain the best colleagues across all relevant disciplines. The NHS Consultant Practitioner and Capability framework has provided a structured and robust training framework which enables colleagues, from all disciplines, the opportunity to remain patient-facing throughout their career, providing the multidisciplinary clinical leadership our service needs. The colleagues who have moved into these roles, and who are currently training to do so, are expert, experienced and highly valued.</p>\r\n
\r\n
<p>A consequence is that the term ‘Consultant’ is now used by those from multiple professional backgrounds. This is part of a wider expansion of roles across the NHS which has led to a number of job titles that patients may not be fully familiar with. It is important that patients know who is treating them.</p>\r\n
\r\n
<p>Many colleagues, both those who are medical doctors and those who are not, hold either professional or research doctorates. These are substantial achievements. Colleagues are rightly proud of their qualifications and, of course, want to use the title that they have worked so hard for. There is potential, nevertheless, for the term ‘doctor’ to be confusing for patients, when it is used in a clinical setting to denote research or other professional doctorates, rather than someone who is practising as a medical doctor.</p>\r\n
\r\n
<p>The BGS supports the expansion of senior multidisciplinary leadership roles to deliver the best for our patients, but recognises the potential for new roles and titles to cause confusion for the public and the workforce. In order to avoid ambiguity, we suggest members do the following:</p>\r\n

\t<li>Review their job title – is it clear to those who read written correspondence what their clinical background is? ‘Consultant practitioner’ is ambiguous. ‘Consultant physician’, ‘consultant nurse’, or ‘consultant therapist’ is not.</li>\r\n

<div class="default-table">Previously known as anticipatory care, proactive care is one of the three original streams of the Ageing Well programme, as detailed in the NHS Long Term Plan, published in January 2019. The other two streams of the Ageing Well programme, Enhanced Health in Care Homes and Urgent Community Response, have both been fully rolled out across England. However, anticipatory/proactive care has been beleaguered with delays and funding problems.</div>\r\n
\r\n

<div class="default-table"><span style="font-size:10.0pt"><span style="line-height:107%"><span style="font-family:"Verdana",sans-serif">Proactive care is a vital part of helping to delay frailty and maintain older people’s independence. BGS members tell us how important it is to act early rather than waiting until older people are in need of urgent or emergency care. In April last year, </span></span></span><a href="https://www.hsj.co.uk/finance-and-efficiency/leading-clinicians-horrified-as-nhse-slashes-community-funding-without-warning/7032243.article"><span style="font-size:10.0pt"><span style="line-height:107%"><span style="font-family:"Verdana",sans-serif">we raised concerns about funding for this work being cut</span></span></span></a><span style="font-size:10.0pt"><span style="line-height:107%"><span style="font-family:"Verdana",sans-serif"> and we followed this with a </span></span></span><a href="https://www.bgs.org.uk/AntCare"><span style="font-size:10.0pt"><span style="line-height:107%"><span style="font-family:"Verdana",sans-serif">blog series</span></span></span></a><span style="font-size:10.0pt"><span style="line-height:107%"><span style="font-family:"Verdana",sans-serif"> showcasing the difference that these interventions can make for older people. '</span></span></span><a href="https://www.bgs.org.uk/Blueprint"><i><span style="font-size:10.0pt"><span style="line-height:107%"><span style="font-family:"Verdana",sans-serif">Joining the Dots</span></span></span></i></a>'<span style="font-size:10.0pt"><span style="line-height:107%"><span style="font-family:"Verdana",sans-serif">, our blueprint for preventing and managing frailty in older people, highlights proactive anticipatory care as one of seven key touchpoints of care and support for older people across the system. <p><span style="font-size:10.0pt"><span style="line-height:107%"><span style="font-family:"Verdana",sans-serif">We welcome the focus of this guidance on people living at home with moderate or severe frailty, for whom the right support from health and social care professionals, voluntary sector agencies and neighbourhood teams can help to delay further development of their frailty. Frailty affects 1 in 10 over the age of 65 and up to half of the population over 85 and it is these groups who are most likely to benefit from proactive care. Many people with frailty will also be living with other long-term conditions, which makes the focus on providing holistic assessment in this guidance very welcome.</span></span></span></p>\r\n
\r\n
<p><span style="font-size:10.0pt"><span style="line-height:107%"><span style="font-family:"Verdana",sans-serif">However, we remain disappointed that the initial promised funding for this work was cut and has not been reinstated. Integrated Care Systems (ICSs) are struggling with the many commitments they are required to fund. As the recent report from the Chief Medical Officer pointed out, maximising the health and life chances of older people should be considered a national priority. Upstream action to support healthy ageing and proactive care for frailty should be part of the Government’s overall public health strategy with additional funding provided. We urge the Government to allocate funding to allow systems to roll out proactive care. Effective implementation of proactive care will save the NHS money in the long term as it will help older people to remain well without needing costly hospital care.</span></span></span></p>\r\n
